MAC Healthcare Services is currently seeking an experienced Community Support Worker to join their team in a full-time capacity. This role is integral to providing essential assistance and support to individuals within the community, with a focus on fostering well-being and social integration.
Key Responsibilities:
- Deliver community access support, including recreational outings, everyday appointments, and in-home assistance.
- Assess clients’ needs and develop, implement, and evaluate educational, training, and support programs.
- Conduct client interviews to identify specific challenges and tailor support accordingly.
- Evaluate community needs and resources related to health, welfare, housing, employment, training, and other services.
- Raise awareness about available resources and services for individuals and families.
- Engage with families, welfare agencies, government organisations, and private businesses to address community issues.
- Provide counselling and emotional support to clients.
- Facilitate social inclusion by encouraging participation in community activities and events.
- Assist clients in building and maintaining social connections and support networks.
- Prepare submissions for funding and resources, and reports for government bodies and agencies.
- Collaborate with stakeholders to ensure documentation supports ongoing funding and program sustainability.
- Maintain accurate and up-to-date records of client interactions and monitor progress.
- Work closely with other support workers, healthcare professionals, and family members to provide holistic care.
Skills and Experience Required:
- Certificate IV or above in Community Services, Welfare, or Youth Work.
- Minimum of 2 years’ proven experience working with diverse populations, including individuals with disabilities, the aged, or those with mental health challenges.
- High level of interpersonal and communication skills, with the ability to build trust and rapport with stakeholders.
- Solid organisational skills, attention to detail, and the ability to prioritise and meet deadlines.
- Ability to work independently and as part of a team.
- Willingness and ability to travel to visit beneficiaries using own private motor vehicle.
- Current NSW Class C driver’s licence (unrestricted).
Highly Regarded:
- Sound knowledge of government services such as DVA, My Aged Care, and NDIS.
- Current First Aid certificate or willingness to obtain one.
- Experience working within the community sector.
- Previous experience with CRM and online case management tools.
- Knowledge of trauma-informed practice, person-centred practice, and a strength-based approach to service delivery.
- Proficiency with Microsoft Office suite, including Word, Excel, Outlook, and Teams.
Pay: $80,000.00 – $85,000.00 per year
