About the Role

Think community health is just home visits? Think again.

Community care offers far more variety than many people realise. This is a dynamic role where you'll work across home and centre-based settings, collaborate with an experienced multidisciplinary team, contribute to service improvement initiatives, and build meaningful relationships with older adults as they work towards their goals.

Based primarily at our Springvale, Kingston or Dandenong sites, with opportunities to work across Monash Health locations as required, this full-time ongoing role offers variety, collaboration and the opportunity to positively impact the health and wellbeing of older adults living in the community.

In this role, you will:
Deliver individual and group-based interventions in centre-based and home settings
Work under the delegation and supervision of Allied Health Professionals to support client care goals
Collaborate with a passionate multidisciplinary team committed to evidence-based practice
Support clients to maintain their independence, health and quality of life

About You

You're passionate about helping older adults remain independent and enjoy working in a collaborative, multidisciplinary setting. You value person-centred care, communicate effectively and are eager to continue developing your skills.

You'll also bring:
Essential:
Certificate IV in Allied Health Assistance
Minimum three years' full-time equivalent Allied Health Assistant experience
Current driver's licence

Desirable:
Experience working with adults in an inpatient, rehabilitation or community setting
