About the Role

We are seeking a compassionate, reliable, and proactive Support Worker to assist individuals in achieving greater independence, wellbeing, and quality of life. You will provide person‑centred support in the home and community, helping clients live safely, confidently, and with dignity.

Key Responsibilities

- Provide daily living support including personal care, meal preparation, and household tasks
- Assist clients
- to build life skills, confidence, and independence
- Support community access such as appointments, shopping, social activities, and recreation
- Follow individual support plans, goals, and routines
- Promote safety, respect, choice, and empowerment at all times
- Maintain accurate notes, incident reports, and communication logs
- Work collaboratively with families, coordinators, and other professionals

Desirable

- Experience in disability, aged care, mental health, or community support
- Certificate III or IV in Individual Support, Disability, Community Services, or similar
