Direct Care Australia is seeking an experienced and compassionate Registered Nurse to join our growing Community Aged Care team.

Due to continued growth, we are seeking an additional Registered Nurse to join our established Community Aged Care team. This role combines hands-on community nursing with care management, working collaboratively with participants, families and a multidisciplinary team to support older Australians to live safely, independently and with dignity in their own homes.

As our Community Aged Care division continues to grow, we are expanding our clinical capability to ensure participants receive the highest standard of nursing care and support.

About the role

Reporting to the Head of Community Aged Care, you will provide a combination of clinical nursing services and care management support across metropolitan Adelaide.

Your responsibilities will include:

- Conducting comprehensive nursing and clinical assessments.
- Providing wound management, medication reviews and ongoing clinical monitoring.
- Supporting participants living with chronic and complex health conditions.
- Developing, implementing and reviewing individualised care plans.
- Undertaking clinical reviews for Support at Home participants.
- Working collaboratively with Care Partners to coordinate participant care.
- Liaising with GPs,



hospitals, allied health professionals and external service providers.
- Providing education and support to participants, families and carers.
- Maintaining accurate clinical documentation and case notes.
- Contributing to quality improvement initiatives and clinical governance activities.

About you

To be successful you will have:

- Current registration as a Registered Nurse with AHPRA.
- Experience in community nursing, aged care, primary health or rehabilitation.
- Strong clinical assessment and decision-making skills.
- Excellent communication and interpersonal skills.
- A genuine commitment to person-centred care.
- The ability to work independently while contributing positively within a multidisciplinary team.
- Current unrestricted driver's licence.
- National Police Check and relevant worker screening clearances (or willingness to obtain).

Desirable

- Experience with Support at Home or Home Care Packages.
- Care management or case management experience.
- Wound care experience.
- Dementia and chronic disease management experience.
- Knowledge of My Aged Care and current aged care reforms.
