About the Role
Hope & Care Community Services is seeking compassionate, dedicated, and experienced Support Workers to join our team in Brisbane's Northside.
This role involves supporting participants with complex physical disabilities and high-intensity support needs. We are looking for professionals who have the skills, confidence, and commitment to provide high-quality, person-centred care while treating every participant with dignity, respect, and compassion.
We are particularly seeking Support Workers who are available to commit to ongoing permanent rostered shifts, including active overnight shifts, to provide consistency and continuity of care for our participants.
Qualifications & Experience
- Certificate III in Individual Support, Certificate IV in Mental Health, Nursing, or other relevant qualifications (preferred but not essential).
- Demonstrated experience supporting participants with high-intensity support needs.
- Experience providing: Manual handling, Personal care, Hoist transfers, PEG feeding, and supporting non-verbal participants
- Experience supporting participants living with Cerebral Palsy, Dysphagia, Epilepsy, Complex physical disabilities
- Experience implementing Positive Behaviour Support Plans (PBSP) is desirable.
- Strong understanding of person-centred care and maintaining participant dignity.
- Excellent communication and interpersonal skills.
- Current NDIS Worker Screening Check.
- Current Driver Licence and reliable, comprehensively insured vehicle.
- Ability to commit to a minimum of 2–3 shifts per week, including active overnight shifts, with a preference for workers seeking ongoing consistent rosters.
Key Responsibilities
- Deliver high-quality,
person-centred support to participants with complex care needs.
- Provide assistance with personal care, transfers, mobility, PEG feeding, and other daily living activities.
- Safely perform manual handling and hoist transfers in accordance with training and best practice.
- Monitor participants' health and wellbeing and respond appropriately to seizures or other health-related incidents.
- Support participants with communication needs, including those who are non-verbal.
- Maintain accurate progress notes, documentation, and incident reports.
- Collaborate with participants, families, allied health professionals, and the wider support team.
- Follow Positive Behaviour Support Plans where applicable.
- Uphold the NDIS Code of Conduct and Hope & Care Community Services' policies and procedures at all times.

Job Type: Casual
Pay: $45.28 – $63.39 per hour
