About the Role

Chosen Family is seeking highly skilled Nurse Practitioners (NPs) to provide advanced clinical assessment, care planning, and nursing leadership across disability and community services.

The NP will work autonomously and collaboratively to support participants with complex health, behavioural, psychosocial, and disability-related needs.

Key Responsibilities

Advanced Clinical Practice

- Conduct advanced clinical assessments and diagnostic evaluations
- Prescribe and manage medications within scope of practice
- Develop advanced treatment plans and interventions
- Support participants with:
- Chronic health conditions
- Complex disabilities
- Mental health presentations
- Behavioural concerns
- High-intensity support needs
- Provide advanced wound, continence, and chronic disease management
- Coordinate referrals and specialist healthcare supports

Clinical Leadership & Governance

- Provide clinical leadership across nursing and disability support teams
- Mentor Registered Nurses, ENs, and support workers
- Contribute to policy development and clinical governance frameworks




- Lead quality improvement and evidence-based practice initiatives
- Participate in incident reviews and clinical risk management processes

Participant-Centred Care

- Promote participant dignity, autonomy, and informed decision-making
- Deliver holistic, trauma-informed, and culturally safe care
- Support participant wellbeing, independence, and quality of life
- Facilitate collaborative care planning with participants and stakeholders

Stakeholder Engagement

- Liaise with GPs, hospitals, specialists, allied health professionals, and families
- Participate in multidisciplinary care coordination and case conferencing
- Build strong relationships with community and healthcare providers

Compliance & Documentation

- Maintain comprehensive clinical documentation and assessment records
- Ensure compliance with NDIS Practice Standards and clinical governance requirements
- Participate in audits, reporting, and quality assurance activities
- Maintain privacy, confidentiality, and qualified standards

Essential Criteria

- Current AHPRA Registration as a Nurse Practitioner
- Extensive disability, community, or complex care experience
- Current NDIS Worker Screening Check
- Strong leadership and clinical governance skills
- Excellent communication and stakeholder engagement abilities

Desirable Criteria

- Mental health NP endorsement
- Experience within disability health or psychosocial supports
- Community outreach experience
- Experience leading multidisciplinary teams
