Your Key Responsibilities:

- Conduct intake assessments, including pre-tenancy interviews, and manage housing and tenancy waitlists.

- Deliver tailored, trauma-informed case management and develop person-centred plans to support client goals and housing outcomes.

- Provide wraparound support through advocacy, referrals,

and coordination with internal and external services.

- Build strong, culturally safe relationships with clients to understand and respond to their immediate and long-term needs.

- Work collaboratively with the Department of Housing and stakeholders to secure and sustain tenancy outcomes.

- Support tenancy establishment and maintenance, including move-ins, property care, accessing essential services, and NILS loans.

- Maintain accurate and timely case notes, reports, and documentation, contributing to program compliance and service outcomes.
