This role offers the opportunity to work in a community-led model, supporting improved health outcomes for people living with chronic disease in a meaningful and collaborative environment.

What’s the role?
As the ITC Program Care Coordinator, you will support individuals and families living with complex chronic conditions by coordinating care across local and specialist services. You will work closely with the multidisciplinary team to develop and implement personalised care plans, strengthen referral pathways, and improve access to services.

The full-time position will be Darwin based, with a fly-in-fly-out (FIFO) schedule of 2 weeks per month based in Maningrida (Monday to Friday) and the other 2 weeks per month based in Darwin. Other working schedules may need to be negotiated with additional time in community for certain events and if increased client contact is required. Accommodation in Maningrida and flights between Darwin and Maningrida will be subsidised. The contract will be for a period of 12 months with likelihood of renewal for an additional 12 months subject to funding.

What you need to be successful

- Current AHPRA registration as a Registered Nurse

- Minimum 5 years’ post-registration experience

- Strong clinical assessment, care coordination, and communication skills

- Experience in chronic disease management and care planning

- Ability to work collaboratively across multidisciplinary teams and external providers

- Understanding of Aboriginal Community Control, cultural safety, and social determinants of health

Additional requirements:

- Current BLS certification and relevant immunisation training

- Experience working in culturally diverse and/or remote settings (desirable)

- Manual driver’s licence

- National Police Check and NT Working with Children (Ochre Card) or ability to obtain

- Compliance with vaccination and immunisation policies
