Position Overview

We are seeking a reliable, compassionate and person-centred Support Worker to provide community access support to an NDIS participant.

The primary focus of this role is to support the participant to safely access the community, attend appointments, participate in social and recreational activities, complete shopping and errands, and work towards their individual NDIS goals.

The participant may occasionally require limited support within their home, depending on their needs and preferences.

Shifts will generally begin at a minimum of two hours, with the possibility of longer or additional shifts as the participant’s support needs develop.

Key Responsibilities

- Support the participant to access the community safely and confidently.
- Assist with attending medical, allied health or other appointments.
- Support participation in social, recreational and community activities.
- Assist with shopping, errands and accessing local services.
- Provide transport or accompany the participant using approved transport arrangements.
- Encourage independence, decision-making and community participation.
- Provide companionship and emotional support while maintaining professional boundaries.
- Assist with light household tasks when required, such as general tidying, meal preparation or organising personal items.
- Support the participant with daily living activities within the home where requested and within the worker’s training and scope.
- Follow the participant’s support plan, risk assessment, behaviour support strategies and individual preferences.

- Complete accurate progress notes and required documentation following each shift.
- Report incidents, risks, changes in presentation or concerns to management promptly.
- Maintain the participant’s privacy, dignity, confidentiality and choice at all times.

Essential Requirements

- Previous experience working in disability, aged care, community services or a related support role is preferred.
- Current NDIS Worker Screening Check.
- Current Working with Children Check, where applicable.
- Current First Aid and CPR certificates.
- Current Australian driver’s licence.
- Access to a reliable, registered and insured vehicle.
- Ability to complete clear and accurate shift notes.
- Good communication, time-management and interpersonal skills.
- Ability to work independently and follow organisational policies and procedures.
- Understanding of person-centred practice, participant choice and NDIS principles.

Personal Attributes

The successful applicant will be:

- Respectful, patient and compassionate.
- Reliable and punctual.
- Friendly and confident supporting community participation.
- Adaptable and responsive to the participant’s changing needs.
- Committed to supporting independence rather than completing all tasks for the participant.
- Able to maintain appropriate professional boundaries.

Employment Conditions

- Casual position.
- Shifts generally commence from a minimum duration of two hours.
- Hours may increase depending on the participant’s needs, preferences and available NDIS funding.
- Some weekday, evening or weekend availability may be required.
