Arche Health is a not-for-profit community-based organisation that delivers high quality support services to the local community. We deliver the Integrated Team Care (ITC) program which is designed to improve health outcomes for Aboriginal and Torres Strait Islander people living with chronic conditions such as diabetes, cardiovascular disease, chronic respiratory disease, renal disease, cancer and mental health conditions. The program provides care coordination, access to multidisciplinary health services, and culturally safe support to help clients manage their conditions effectively.

We require a AHPO (Full Time) who has excellent communication, community engagement and administration skills to provide program coordination for our busy Aboriginal Health Team as part of the Integrated Team Care (ITC) Program. This position is based in Bentley and works closely with GPs, health practitioners, clients and other key stakeholders in the Perth Metro region (predominately North-West and South-East regions) to achieve better health outcomes for our Aboriginal and Torres Strait Islander clients through better access to culturally appropriate health services.

ROLE The Aboriginal Health Project Officer (AHPO) will assist and support the Arche Health Aboriginal Health Manager to improve health outcomes for Aboriginal and Torres Strait Islander people living with chronic health conditions through better access to high quality, culturally appropriate, coordinated care through the Integrated Team Care (ITC) Program. The position will work collaboratively with members of a multidisciplinary team, General Practitioners, and other internal and external medical professionals and stakeholders to ensure the ITC Program is delivered in an efficient and qualified manner,



in line with the contractual requirements.

DUTIES & RESPONSIBILITIES

- Work as a senior team member in promoting the ITC program delivery in conjunction with the Aboriginal Health Manger;
- Supporting Care Coordinators and Outreach Workers where required;
- Promote the ITC Program to Aboriginal and Torres Strait Islander client’s and community in line with the ITC program guidelines;
- Develop and implement strategies to improve the capacity of mainstream primary care providers to deliver culturally appropriate primary care services to Aboriginal and Torres Strait Islander people, including promoting self-identification; uptake of ATSI MBS items (715 – Health Assessments);
- Collaborate with local Aboriginal health services (ACCHSs) and mainstream health services in a partnership approach for the delivery of primary care services;
- Identify and address barriers faced by Aboriginal and Torres Strait Islander people when accessing mainstream primary care services, including but not limited to primary care, pharmacy, allied health and specialists;
- Provide support to mainstream primary care providers in understanding the needs and barriers of Aboriginal and Torres Strait Islander clients, including supporting them to self-identify their Indigenous status when accessing mainstream primary care services;
- Deliver or coordinate cultural awareness training and quality improvement activities;
- Coordinate relevant health education events;
- Develop and disseminate resources for Aboriginal and Torres Strait Islander people about accessing services and managing chronic disease;




- Assist with program and service coordination;
- Contribute to developing culturally appropriate health promotion material;
- Liaise and collaborate with hospital and health care staff, GPs, Arche Health staff and other health providers to improve the delivery of culturally appropriate care for clients;
- Contribute as an effective team member at Arche Health team meetings and represent Arche Health on appropriate committees as required.

SELECTION CRITERIA – QUALIFICATIONS, SKILLS, KNOWLEDGE, EXPERIENCE

- Possession of tertiary qualification in health discipline; highly preferred.
- Aboriginality; highly preferred.
- Demonstrated knowledge and understanding of Aboriginal culture and family structures and the ability to engage and build strong working relationships with a culturally and linguistically diverse range of clients and stakeholders.
- Demonstrated knowledge and understanding of Aboriginal culture and health issues affecting Aboriginal communities.
- An understanding of the physical and emotional impact of chronic disease on individuals and families
- Highly developed communication skills, both verbal and written, with an ability to interact with a diverse range of people i.e. health professionals, service providers.
- Experience in contributing to service development.
- Demonstrated ability to function as a team of a multidisciplinary team.
