About the opportunity

As Care Finder, you will play a meaningful role in supporting senior Australians who need intensive assistance to understand, navigate and access aged care services. You will build trusted relationships, connect people with practical community supports, and help them make informed choices that promote independence, dignity and wellbeing.

This full-time role also offers working-from-home flexibility, supporting a balanced and responsive way of working.

In this role, you will apply a person-centred approach that upholds dignity, respect, choice and self-determination, while working in line with Care Finder policy and guidelines.

Your focus will include

- Undertaking holistic assessments and supporting planning to determine and document individual needs and access to supports.
- Ensuring that individuals are valued and respected and that supports aim to promote independence and client self-determination.
- Visiting clients in home, engaging clients in different environments, and informational outreach to service providers and the community.
- Meeting with providers to arrange services and connecting with other relevant supports in the community.




- Organising and participating in case conferences and interagency meetings in a collaborative and consultative manner.
- Promoting Footprints programs and services and advocating for service options for the target group, in the wider community, through sound working relationships with partners and networks within government, non-government and private sectors.
- Undertaking administrative functions, including maintenance of client data base, maintaining a high standard of client files, reporting requirements, and data input activities.

What you will bring:

- Experience working with a diverse client group including older people, people with disabilities, people experiencing mental illness, and people experiencing homelessness.
- Demonstrated experience in community networking and working collaboratively with external service providers to plan and provide consistent, responsive, and appropriate client focused services.
- Local community connections with the Care Finder target population or a specific sub-group within the care finder target population.
- Ability to attain a detailed understanding of the range of aged care supports and services available in the local community, the process to access, and any eligibility requirements for other relevant supports.
- Computer literacy and the ability to effectively manage a client data base.





Requirements:

- Relevant tertiary qualification and/or relevant experience.
- Current QLD Driver's Licence and access to registered vehicle.
- NDIS Worker Screening (Yellow Card) and Blue Card.
- Current First Aid and CPR Certificate.
