The position
At VMCH, our Care Partners are at the heart of supporting older Australians to live life their way. We're looking for an organised and caring qualified to manage a portfolio of Home Care Package clients across Melbourne's Northern and Eastern Metropolitan regions. If you're passionate about building trusted relationships and empowering people to remain independent at home, we'd love to hear from you.
In this role you will:
- Conduct assessments and develop person-centred care plans
- Coordinate services and monitor client outcomes
- Support clients and families to make informed care decisions
- Complete reviews and maintain accurate documentation
- Ensure compliance with aged care quality standards
- Collaborate with internal teams and community providers
About you

What’s required
- Diploma qualification in a relevant field or higher (e.g. Diploma, Advanced Diploma, Bachelor's degree or above)
- Experience as a Care Partner or Case Manager within Support at Home, Home Care Packages
- Excellent communication and computer skills
- Understanding of aged care reforms and quality standards
- Ability to work autonomously and manage competing demands
- Commitment to customer service, cultural diversity, and workplace safety
- Willingness to travel as required and a current Victorian driver’s licence
- Satisfactory National Police Check
- Knowledge of Carer Recognition Acts and relevant service frameworks
