Role Overview We're looking for a compassionate and capable Community Care Link Worker to join our growing Community Care team. Working alongside our Community Care Coordinator, you'll support people experiencing life's challenges by providing practical support, encouragement, community connection and pathways into local services. Rather than long‑term case management, this role focuses on walking alongside people, helping them build confidence, strengthen relationships and reconnect with community.
