What You'll Be Doing

- Coordinating personalised care plans that empower clients to achieve their goals.
- Managing complex care arrangements across NDIS, TAC and iCare programs.
- Working closely with clients, families, support workers and allied health professionals to deliver seamless, high-quality care.
- Coordinating services, rostering supports and responding to changing client needs.
- Advocating for clients to ensure they access the right funding, services and community resources.
- Maintaining accurate documentation and ensuring compliance with industry standards.
- Providing regular updates to clients, families and internal stakeholders.
- Problem-solving, thinking on your feet and making every day count.

What You'll Bring

We're looking for someone who enjoys being busy, takes pride in delivering outstanding customer service, and knows how to keep multiple priorities moving.

You'll have

- Experience in disability,



attendant care or community care coordination.
- A solid understanding of NDIS, TAC and/or iCare programs.
- Exceptional organisational, coordination and rostering skills.
- Outstanding communication and relationship-building abilities.
- A proactive, solutions-focused mindset with the confidence to make decisions.
- The ability to manage competing priorities without losing attention to detail.
- Experience supporting clients with complex or high-needs care (highly regarded).
