Role Description This is an on-site Disability Support Worker role based in Wangaratta, VIC, offered on a contract basis.

The Disability Support

Worker will support clients with daily living activities such as personal care, meal preparation, medication prompts, and community participation.

Responsibilities include implementing individual support plans, promoting independence, and assisting clients to access local services, appointments, and social activities. The role involves maintaining accurate documentation, reporting changes in client needs, and working closely with families, health professionals, and partner services. The worker is expected to uphold safe work practices, follow organizational policies,



and contribute to a respectful, inclusive workplace.

Qualifications

- Experience in disability support, community services, or a related care role, with a person-centered and strengths-based approach.
- Strong interpersonal, communication, and active listening skills to build trust and rapport with clients, families, and colleagues.
- Ability to assist with personal care, daily living tasks, and behavioral support while maintaining dignity and respecting individual preferences.
- Organizational and time-management skills to follow support plans, complete documentation, and respond appropriately to changing needs.
- Understanding of safeguarding, duty of care, and basic knowledge of relevant disability and community services frameworks.
- Certificate III or IV in Disability, Community Services, Individual Support, or a related qualification (or progress toward such qualifications).
- Current First Aid and CPR certification, valid driver’s license, and ability to meet any required background or safety checks.
- Capability to work shifts, including evenings, weekends, and public holidays, as required in a residential support environment. Sleepovers and Weekends as a must
