Open Sky Disability Services is seeking experienced and reliable Disability Support Workers to join our growing team across Perth.

We are currently recruiting support workers for:

South of Perth

North of Perth

Baldivis

Mandurah

Surrounding suburbs across Perth

About the Role The role involves supporting participants with daily living activities, personal care, community access, transport, medical appointments, social participation, medication support, household tasks, and implementation of person-centred support plans.

We are looking for professional, flexible, respectful, and skilled support workers who can provide secure, high-quality support to people with disability.

Essential Requirements

Applicants must have

Valid driver’s licence

Own reliable vehicle

Current NDIS Worker Screening Check

Current Working with Children Check, where required

Current First Aid and CPR

Manual Handling training or willingness to complete

Medication competency or willingness to complete

Good communication and documentation skills

Ability to follow care plans, behaviour support strategies, and workplace procedures

Previous disability support experience preferred

Severe dysphagia support experience

Advanced mealtime management training

Epilepsy management experience

Complex bowel care experience

Stoma care experience

Experience supporting participants with complex needs

Experience working in SIL or community support settings

Key Responsibilities

Provide safe, respectful, and person-centred support

Assist with personal care and daily living tasks

Support participants with community access and appointments

Assist with mealtime support as per individual plans

Follow medication, health, behaviour, and support plans

Complete accurate progress notes and shift documentation

Report incidents, risks, and changes in participant needs promptly

Maintain professional boundaries and confidentiality

Promote participant choice, independence, dignity, and safety

Availability

Flexible availability is preferred, including:

Morning shifts

Afternoon shifts

Evening shifts

Inactive/ Active Sleepover Shift

Weekend shifts

Short-notice cover

Ongoing regular shifts
