What You'll Be Doing:
- Community Case Management: Conduct home reviews, risk assessments, and individualised care plans under the 2026 Support at Home program.
- Workforce Support:



Guide and supervise local Health Care Workers (HCWs) for protected care delivery.
- Multi-Disciplinary Collaboration: Partner with local GPs, Allied Health, and participant families.
- Budget & Compliance: Monitor funding allocations and maintain documentation (TRAKCare/RiskMan).

What You'll Need to Apply:
- Qualifications: Degree/Diploma in Case Management, Social Work, Psychology, Nursing, or Allied Health.
- Experience: Background in community aged care, CHSP, or NDIS support coordination.
- Mandatory Checks: Valid VIC Driver's Licence, National Police Check / NDIS Clearance, and current Flu Vaccination.
- Working Rights: Full Australian working rights (sponsorship unavailable).
