Key Responsibilities - Provide efficient clinical pharmacy services to inpatients or outpatients.

- Conduct pharmaceutical admission and discharge reconciliations, monitor medication outcomes such as therapeutic drug monitoring, to ensure quality use of medicine is upheld.
- Work cooperatively and effectively within a multidisciplinary team and provide advice regarding patient care. What You’ll Bring Essential requirements:
