An exciting chance has arisen for an experienced Social Worker to join the Adults 1:1 Support & Review Team in Beverley. This full-time ongoing locum role offers the chance to make a meaningful impact by ensuring adults living in residential care receive person-centred support that meets their assessed needs.

What you will do:
- Complete Care Act 2014 assessments, reviews, and support plans.
- Review existing residential care packages to ensure they continue to meet assessed needs.
- Evaluate the necessity and effectiveness of 1:1 support through evidence-based assessments.
- Undertake risk assessments and develop comprehensive risk management plans.
- Complete Mental Capacity Assessments and make Best Interest decisions where appropriate.
- Apply safeguarding principles and respond appropriately to safeguarding concerns.
- Work collaboratively with Review Teams, Contract Monitoring, Quality Assurance, and Commissioning colleagues to achieve positive outcomes.
- Promote independence, choice, and wellbeing through a strengths-based approach.
- Attend meetings and reviews in person to contribute to effective assessment and decision-making.
Qualifications/Requirements:
- Social Work England registration is essential.
- Recognised Social Work qualification.
- Experience within Adult Social Care, including Care Act assessments and reviews.
- Knowledge of the Care Act 2014, Mental Capacity Act, and Adult Safeguarding legislation.
- Strong assessment, communication, and partnership-working skills.
