About the Role

As Registered Nurse – Aged Care & NDIS, you’ll deliver high-quality clinical care and care coordination services to older Aboriginal and Torres Strait Islander people and NDIS participants across Yalata. Working across community, clinic, and home environments, you’ll help clients remain healthy, independent, and connected to family, culture, and Country through a holistic and strengths-based approach to care.

Key Responsibilities

- Conduct comprehensive clinical assessments and reviews.
- Develop and implement person-centred care plans.
- Provide medication, wound, chronic disease, and clinical management.
- Coordinate care with GPs, specialists, allied health providers, hospitals, and community services.
- Support clients to navigate aged care and NDIS services.
- Deliver health education and self-management support.
- Promote culturally safe, trauma-informed care practices.
- Participate in quality improvement, accreditation, and clinical governance activities.

About You





You are a caring and adaptable nurse who enjoys building relationships and making a meaningful impact in people’s lives.

Essential Requirements

- Current Registered Nurse registration with AHPRA.
- Minimum two years’ nursing experience in aged care, disability, primary health care, or community nursing.
- Strong clinical assessment and care planning skills.
- Understanding of Aged Care Quality Standards and NDIS Practice Standards.
- Current Driver’s Licence.
- Current CPR certification.
- Ability to work independently and as part of a multidisciplinary team.

Highly Regarded

- Experience within an Aboriginal Community Controlled Health Organisation.
- Experience working in rural or remote communities.
- Knowledge of aged care, NDIS, and community care programs.
- Understanding of culturally safe and trauma-informed practice.
