We're looking for a compassionate Registered Nurse | Clinical Care Partner to join our growing Support at Home team. While this role is ideally full-time, we're happy to discuss flexible part-time arrangements for the right candidate.

Based from our Chermside office, you'll enjoy a flexible hybrid work model, working across home, office and the community while supporting older people throughout North Brisbane and Moreton Bay.

Working alongside an experienced multidisciplinary team, you'll combine your clinical expertise with care coordination to help clients remain safe, independent and connected in the place they most want to be – home. Whether your background is community nursing, residential aged care, general practice or another clinical setting, this role offers greater autonomy, flexibility and variety while making a genuine difference every day.

About the role

As our Registered Nurse | Clinical Care Partner, you'll:

- Act as the primary contact and advocate for a portfolio of community aged care clients (maximum 50 clients)
- Undertake clinical assessments, home visits, health checks and medication management within your Registered Nurse scope of practice
- Develop, coordinate and review person-centred Care Plans aligned with clients' goals, health needs and funding
- Support clients receiving restorative care, chronic disease management and end-of-life care
- Coordinate services with clients, families, GPs, allied health professionals, Support Workers and other community providers
- Provide clinical guidance, mentoring and day-to-day support to Support Workers and colleagues
- Monitor client funding and budgets to ensure services remain aligned with Support at Home funding
- Contribute to quality improvement, clinical governance and continuous service improvement
- Maintain accurate documentation and ensure compliance with aged care legislation, funding requirements and Quality Standards
- Participate in an occasional on-call roster

About you





You'll be a Registered Nurse who enjoys building relationships just as much as delivering excellent clinical care.

You'll also bring

- Current AHPRA registration as a Registered Nurse
- Current First Aid and CPR Certificate
- Current Australian Driver Licence
- Current National Police Check (or willingness to obtain)
- Excellent communication and relationship-building skills
- Confidence working autonomously while collaborating as part of a multidisciplinary team
- Strong organisational, problem-solving and time management skills
- High level of computer literacy and experience using clinical or care management systems
- A genuine commitment to delivering compassionate, person-centred care

Experience in community nursing, general practice or residential aged care will be highly regarded.

Experience managing aged care budgets and undertaking client risk assessments will also be highly regarded.
