Use your clinical expertise to support people with complex mental illness through coordinated, recovery-focused care. The OpportunityCatholicCare is looking for an experienced and compassionate Clinical Coordinator to join our Mental Health team in Muswellbrook. This permanent part-time role, working three days per week, supports people with severe and complex mental illness through coordinated, person-centred clinical care. In this role, you will provide trauma-informed and recovery-focused support based on each client’s GP Mental Health Treatment Plan or Psychiatrist Care Plan. You’ll work closely with our Peer Support Worker to ensure clients feel supported, empowered and connected throughout their mental health journey. To be successful in this role, you will be a Registered Mental Health Nurse, Psychologist or Social Worker or Occupational Therapist with mental health experience or hold an equivalent qualification. Mental health accreditation is desirable. You will also collaborate with GPs, psychiatrists, psychologists, allied health professionals and family or support people to ensure clients receive the wrap‑around care they need.This opportunity is ideal for a skilled clinician who values autonomy, teamwork and making a meaningful difference. POSITION DESCRIPTION
The aim of the Clinical Care program is to provide greater access to mental health services for those people living with mental illness in the HNECC region. Mental Health Services in Primary Care will improve service integration and achieve alignment with a person centred, stepped care approach. The aims will be achieved by engaging a range of mental health professionals to provide a suite of evidence based mental health services to people that would otherwise have little or no access to such services. The role of the Clinical Coordinator is to provide coordinated clinical care for people with severe and compex mental illness. The care will be delivered in line with a GP Mental Health treatment plan or equivalent, developed by a GP or Psychiatrist. The Clinical Coordinator will work with the Peer support worker to deliver person centred holistic support to clients on their mental health journey. The Clinical Coordinator will collaborate with GP’s, psychiatrasts, and other allied health professionals,
as well as family members or support people to ensure that clients are able to move forward on their journey with the support they need.
