You will provide comprehensive assessments, clinical recommendations, and tailored therapy plans to support activities of daily living, mobility, transfers, and skin integrity management.

You will contribute to an creative Plena Care Pathway, delivering evidence-based mobility and functional programs, care plan reviews, and both one-to-one and group aging
