Western Health in Melbourne's western suburbs is seeking a Team Leader Registered Nurse Grade 4 to join the St Albans Community Care Unit. You will lead a multidisciplinary team, contribute to clinical meetings, and ensure clinical standards of care are met.

You will assess referrals, provide evidence-based interventions for mental illness, perform comprehensive risk assessments, and review consumers' physical health as part of holistic care.
