What You'll Do

Working across your local region,



you'll undertake comprehensive aged care assessments in clients' homes, supporting older Australians to access services and maintain their independence.

You'll:

- Conduct holistic, person-centred aged care assessments
- Develop support plans focused on wellness, independence, and reablement
- Identify appropriate supports, services, assistive technology, and community connections
- Utilise the Integrated Assessment Tool and My Aged Care systems
- Work collaboratively with clients, families, healthcare professionals, and service providers
- Manage your own workload while delivering high-quality outcomes

About You

You're a qualified clinician who enjoys working independently while making a genuine difference in people's lives.

You'll bring:

- Experience in aged care, community health, disability, health, or social services assessments
- Robust assessment, planning, and clinical reasoning skills
- Excellent communication and stakeholder engagement abilities
- Confidence working autonomously and managing competing priorities
- Strong technology and documentation skills
- Existing knowledge of My Aged Care and aged care assessment frameworks (highly regarded)

Highly Desirable





Candidates who have completed MAC Learning and currently hold a Clinical Aged Care Needs Assessor Certificate will be highly regarded.

Essential Requirements

- Registered Nurse, Occupational Therapist, Physiotherapist, or Social Worker qualification
- Current AHPRA registration or AASW membership
- Minimum 12 months' experience within aged care
- Current Clinical Aged Care Needs Assessor Certificate
- Unrestricted Australian Driver's Licence
- Current Influenza Vaccination
