Western Health is seeking an experienced Emergency Department Care Coordinator to join the EDCC Team on a 12-month fixed-term, full time basis. The role focuses on improving patient outcomes by reducing avoidable hospital admissions and coordinating care for patients with complex needs in the Emergency Department. You will work in a multidisciplinary team, develop individualised management plans, and support safe discharges with trauma-informed, person-centred care.
