What you will do
- Work collaboratively with locality hub coordinators and multidisciplinary teams across the area’s major acute hospitals to deliver effective reablement services.
- Undertake home visits to assess individuals’ functional abilities, rehabilitation needs, and support requirements.
- Support service users in achieving greater independence through personalised, goal-focused intervention plans.
- Carry out comprehensive occupational therapy assessments within community settings.
- Recommend and coordinate appropriate equipment, adaptations, and support to maximise safety and independence.
- Work closely with service users and their families to agree achievable goals and positive outcomes.
- Maintain accurate, detailed, and timely case notes, assessments, and reports from both office and home-working environments.
- Monitor progress and review interventions to ensure services remain effective and responsive to individual needs.
- Participate in multidisciplinary meetings and contribute to coordinated care planning.
Requirements
- Current HCPC registration as an Occupational Therapist.
- Full UK driving licence and access to a vehicle are essential.
- Experience within community-based Occupational Therapy, reablement, rehabilitation, or discharge planning environments is desirable.
- Strong assessment, communication, and care planning skills.
- Ability to work independently while contributing effectively within multidisciplinary teams.
