Employment Status: Permanent, full time

Days & Hours of Work: Monday – Friday, 38 hours per week

Competitive Salary: Band 5 - $83,038.80 + 12% superannuation

Location: Council Office in Sandringham, with some flexibility to work from home

About the Role

The Community Care Support Officer assists Team Leaders to lead a team of Support Workers to deliver a coordinated range of in‑home client services and to establish, monitor and review services in response to clients’ changing needs.

Responsibilities

- The leadership, coordination, support and supervision of Support Workers
- The monitoring and review of ongoing and changing needs of clients and their carers
- Actioning service referrals received from external assessment services including completion of service priority of access, an agreed service plan, OH&S; plan and establishment of service fee(s)

Key Selection Criteria

- Tertiary qualification in a management, health, allied health, social science or related discipline or substantial experience working with older people in a community setting




- Knowledge and understanding of aged care and disability services and programmes
- Intermediate skills in Microsoft Office applications
- Previous experience with client management and rostering software – an advantage
- Understanding of the needs of and issues affecting older people, people with disabilities and their carers
- Ability to communicate with older people, people with disabilities and their carers
- Ability to respond to staff and client issues in a logical and considered manner

Mandatory Pre‑Employment Requirements

- Willingness to undergo and complete a satisfactory criminal history check
- Willingness to obtain an employment‑based Working with Children check, or provision to submit one
