Position Details:

Responsibilities include:

- Providing evidence-based assessments, recommendations, and active interventions in clients' homes.
- Supporting hospital substitution programs (Rehab at Home, Hospital in the Home): Acute and sub-acute rehabilitation.
- Participating in government aged care programs (Home Care Package, Short Term Restorative Care, Commonwealth Home Support): helping seniors stay well and independent.
- Working independently in the community and collaborating with a multidisciplinary team.
- Engaging in clinical supervision with a designated supervisor.

Additional requirements:

- Preferred: Community or hospital physiotherapy experience.
- Qualifications: Physiotherapy degree with unrestricted AHPRA registration.
- Must have a valid driver’s license and insured, roadworthy vehicle.
