Are you passionate about helping older people live independently, safely, and with dignity in their own homes? We are looking for a caring, reliable, and compassionate Care Partner to join our Support at Home team and help deliver exceptional care and support to older Australians in the community.

About the Role

As a Care Partner, you will provide personalised support that enables older people to remain connected to their communities and continue living in the comfort of their own homes. The Care Partner will work closely with Support at Home Clients to maximise usage of their Support at Home Funding allocation, coordinate care and services that meets their care needs, goals and preferences. You'll build meaningful relationships with clients and empower them to maintain their independence, wellbeing, and quality of life.

Key Responsibilities

- Work with clients to develop care plans and determine their needs, goals and preferences

- Liaise with associated providers as per client preference to provide care and services as per care plan

- Liaise with internal stakeholders to coordinate high-quality service delivery.

- Advocate for client needs with My Aged Care, Department of Health, Disability and Ageing

- Work With associated providers to deliver high-quality services.





- Support clients to understand their rights and the Support at Home system

About You

We are ideally looking for an Enrolled Nurse, or someone with similar qualifications, who has experience coordinating care for clients living independently in the community and accessing a range of support services, including aged care programs.

The successful applicant will have a strong understanding of how Support at Home funding is applied and managed to ensure clients receive the services they need while remaining within their approved budgets. They will possess sound knowledge of the Aged Care Quality Standards and understand how these standards are applied in a Support at Home setting to deliver safe, effective, and person-centred care.

Experience navigating Support at Home program requirements, interpreting program guidelines, and ensuring services are delivered in a compliant manner is essential. In addition, you will have well-developed customer service skills, with the ability to build positive relationships, respond to client needs, and provide a high standard of support to older people, their families, and other stakeholders.
