What we are looking for:

As a Care Partner, you will be responsible for coordinating, monitoring, and reviewing person-centred home care services for older Australians receiving services under the Support at Home (SAH) and Commonwealth Home Support Program (CHSP). Working within the Strengthened Aged Care Quality Standards and Aged Care Act 2024, you will support participants to maintain their independence, wellbeing, choice, and control while living safely in their own homes.

What will you do:

- Build strong participant relationships through assessments, reviews, care planning, and regular contact to ensure services align with individual goals, needs, preferences, and rights.
- Coordinate and monitor services ensuring supports remain responsive, effective, culturally appropriate, and delivered within approved funding and program requirements.
- Promote participant choice and rights, supporting informed decision-making, access to advocates, community services, and culturally safe, inclusive care.




- Manage quality and compliance by maintaining accurate documentation, care plans, case notes, participant reviews, workplace safety assessments, and quality records in AlayaCare.
- Identify and respond to risks, escalating clinical concerns appropriately and participating in incident management, complaints handling, and safeguarding activities.
- Collaborate with participants, families, Home Care Workers, health professionals, and service providers to deliver coordinated, high-quality care and support.
- Contribute to continuous improvement, audits, career development, service growth initiatives, and the delivery of high-quality aged care services aligned with the Strengthened Aged Care Quality Standards.
- Participate in the On-Call and Duty Care Partner rosters in line with business requirements.

What you will need:

- Certificate IV (or higher) in Aged Care, Community Services, Nursing, or a related field.
- Experience in aged care, community services, care coordination, or case management.
- Knowledge of the Aged Care Act 2024, Participant Directed Care, and person-centred care practices.
- Current National Police Check, Blue Card, First Aid, and CPR certificates (or willingness to obtain).
- Current Driver's Licence and reliable vehicle.
- Strong communication, relationship-building, and organisational skills.
- Sound computer skills, including Microsoft Office and client management systems.
- Experience with AlayaCare, SAH, CHSP, CALD communities, or a second language will be highly regarded.
