We are looking for a Care Partner who loves building trust with clients and families and enjoys coordinating the right mix of services, plans, and supports. You will work alongside clients to understand their goals, explain how funded aged care services work, and coordinate services that match their needs and preferences.

About the role

As a Care Partner, you will be the main point of contact for clients and their chosen support. You will coordinate services, maintain care plans and agreements, and support clients to understand their options and make informed choices. This role suits someone who enjoys balancing meaningful client relationships with the structure of documentation, service coordination and budget monitoring.

What you will do

• Build rapport with clients and their family or registered supporter, and keep communication clear and respectful.
• Explain funded aged care services and what the client is eligible to receive, including service lists, support plans and how care management works.
• Develop, maintain and review the client’s care plan and service agreement, and document changes as needed.
• Coordinate services in line with the client’s goals, preferences, and approved supports, including preferred workers and service times where possible.




• Monitor and manage the client’s budget to ensure services remain appropriate, sustainable, and aligned to funding and contribution limits.
• Liaise with internal teams and relevant health practitioners to support continuity of care and timely responses to changing needs.
• Record care management activity and maintain accurate, respectful documentation in line with required processes.

What you will bring

• Experience in aged care, community services, care coordination or case management.
• Confidence having supportive, conversations with clients and families.
• Strong organisation skills, you can juggle plans, documentation, and follow-ups.
• Comfortable working with budgets and service planning.
• Clear written and verbal communication and confident system use.
• A current C class driver’s licence (travel is required within the Collie area)
• Willingness to complete required checks relevant to aged care work (Police Clearance) as well as annual influenza vaccinations.

Required Skills:

- Certificate III in Community Services or Diploma.
