About This Role
Working closely with our Case Managers and multidisciplinary team, you'll play a key role in coordinating safe, seamless and patient-centered transitions of care. As a Discharge Planner, you'll work with patients, families and external healthcare providers to develop personalised discharge plans that support each patient's goals and ongoing care needs.

Key Responsibilities
- Develop personalised discharge and continuing care plans tailored to each patient's individual needs.
- Coordinate transitions of care by liaising with patients, families, carers and external providers, including rehabilitation, aged care and palliative care services.
- Advocate for patients to ensure their physical, emotional, cultural and healthcare needs are considered throughout the discharge planning process.
- Provide education and resources to patients, carers and staff to support secure and informed care transitions.
- Collaborate with the multidisciplinary team to facilitate timely discharge planning and continuity of care.
- Maintain accurate documentation in accordance with hospital policies and qualified standards.
About You
- Experience in discharge planning, case management or care coordination within a healthcare setting.
- Excellent communication and interpersonal skills, with the ability to build effective relationships with patients, families and healthcare providers.
- Strong organisational and problem-solving skills, with the ability to manage competing priorities.
- A collaborative approach and commitment to delivering high-quality, patient-centred care.
- Current AHPRA registration
