We are seeking an experienced and passionate NDIS Support Coordinator to join our team on a permanent part-time or full time basis.

About the Role:
As an NDIS Support Coordinator, you will:
-Assist participants to understand and implement their NDIS plans.
-Connect participants with appropriate supports and community services.
-Build participants' capacity to achieve their goals and increase independence.
-Coordinate services and advocate for participants when required.
-Maintain accurate case notes and documentation in line with NDIS Practice Standards.
-Develop and maintain strong relationships with participants, families, service providers, and key stakeholders.
Essential Requirements:
To be successful in this role, you must have:
-Minimum 2 years' experience working as an NDIS Support Coordinator.
-Relevant TAFE or tertiary qualification in Community Services, Social Work, Disability, Psychology, or a related field.
-Current Working With -Children Check (WWCC).
-Current National Police Check.
-Current NDIS Worker Screening Check.
-Excellent communication, organisation, and problem-solving skills.
-Solid knowledge of the NDIS, its legislation, and the local service network.
-Current Australian Driver's Licence and reliable vehicle
