What you’ll be doing
As a Triage Scheduler, you will play a critical role in managing incoming aged care assessment referrals and supporting efficient service delivery.

You will be responsible for the initial triage and prioritisation of referrals, ensuring customers are directed to the most appropriate assessment pathway.

You will coordinate workflows by allocating assessment tasks via digital systems (Power App),



maintaining visibility of workloads, and ensuring timely, equitable access to services.

Key Responsibilities

- Review and triage incoming referrals to determine appropriate assessment pathways
- Assess eligibility for aged care services and escalate where required
- Allocate assessments to assessors, ensuring balanced workloads and KPI adherence
- Monitor workflow, track progress, and identify bottlenecks or resourcing issues
- Maintain accurate records and data across systems in line with policy and guidelines
- Collaborate with internal stakeholders to support timely, high-quality service delivery
- Use dashboards and reporting tools to optimise workflow and performance
- Contribute to continuous improvement of triage and allocation processes
- As required, conduct assessments or support plan reviews via phone, telehealth, or in person

This is a Permanent Full-Time position, working Monday to Friday.

You’ll thrive in this role if you

- Bring warmth, professionalism, and clinical insight to every interaction
- Hold a tertiary qualification in a health-related discipline (e.g. nursing, OT, physio, social work)



and current AHPRA registration or eligible to be part of a relevant professional association.
- Have a strong understanding of the Aged Care Act and aged care pathways
- Communicate clearly and kindly, especially when navigating complex or urgent situations
- Are confident using technology, including the My Aged Care portal
- Work well independently but thrive in a collaborative team
- Are flexible, responsive, and ready to adapt to changing needs
- Hold a current First Aid & CPR certificate, driver’s licence, and are willing to use your own vehicle for work
- Have experience working across cultures and with vulnerable communities
