ABOUT THE ROLE

Evergreen’s Support at Home service provides personalised in-home care that supports individuals to remain independent, safe and comfortable in their own homes. It offers a range of flexible services, including clinical care, personal care, domestic support, allied health and social support – tailored to individuals needs and goals.

The service is focused on making daily life easier and enabling people to stay at home for longer with the right level of support.

We are seeking dedicated Registered Nurses to join our team in a permanent part-time capacity for two (2) days per week (Wednesday/Thursday).

You will be required to:

· Conduct clinical assessments and reviews to identify client needs, risks and required supports.

· Develop, implement and monitor individualised care plans to support client wellbeing and independence.

· Coordinate and deliver clinical care, including wound management, medication management,



chronic disease management and complex care interventions.

· Monitor, document and evaluate client outcomes, ensuring timely completion of assessments, care plans, progress notes and incident reports.

· Respond to and escalate clinical risks, deterioration, safeguarding concerns and other emerging health issues.

· Provide clinical guidance and education to support workers, clients and families.

· Liaise with clients, families, GPs, hospitals, allied health professionals and other providers to ensure coordinated care.

· Support compliance with clinical governance, aged care standards, WHS, infection prevention and control requirements, and documentation obligations.

· Maintain clinical records and contribute to quality improvement, regulatory reporting and funding requirements.

ABOUT YOU

You are a caring, reliable and motivated individual who is passionate about supporting older people to live with dignity and independence. You bring a person-centred approach to care and take pride in delivering high-quality support.

You will possess:

· At least 1 years' experience as a Registered Nurse (RN)

· Hold a current registration with AHPRA (Division 1)

· Valid Driver’s licence and willingness to travel for client visits





· Hold a current National Criminal Record Check (or willingness to obtain one)

· Willingness to participate in a pre-employment medical assessment

· Hold a current CPR and First Aid Certificate

· Strong interpersonal and communication skills, with the ability to build positive relationships with Clients and families.

· A commitment to client privacy, confidentiality, and safe work practices
