About the Role:

Echuca Regional Health (ERH) is seeking an experienced and motivated Clinical Nurse Consultant – Complex Care to join our team for a fixed term leave coverage role. This senior clinical role provides high‑quality, person‑centred care coordination for clients with chronic and complex health needs, supporting health independence and reducing avoidable hospital presentations.

As the Clinical Nurse Consultant – Complex Care, you will manage a wide-ranging caseload across the Health Independence Program (HIP), delivering advanced nursing care, care coordination, and health coaching to clients in the community. Working collaboratively within the Complex Care team and with multidisciplinary internal and external partners, you will support clients to achieve self‑management goals and optimise health outcomes through evidence‑based care pathways.

Key responsibilities include:

· Provide advanced nursing care to people recovering from a medical illness or with chronic and complex health needs receive appropriate services to maximise their health

· Support patient intake and triage, conduct assessments,



and coordinate goal-directed care planning in a multi-disciplinary setting to ensure smooth transitions from ERH into the community

· Provides health literacy and self-help coaching to clients with chronic disease in the community

· Assisting the IPAC Team to achieve effective and efficient management of personnel, material and financial resources

· Demonstrates proficiency in the use and purpose of the Remote Patient Monitoring system and devices

· Maintain Department of Health Key performance Indicators for the Complex Care program under the HIP Guidelines

Qualifications/experience required:

· Registered Nurse with AHPRA

· Demonstrated experience in chronic disease management and complex care coordination

· Post graduate qualifications in relevant field

· Ongoing CPD in Chronic Disease Management
