Jobtailor is seeking a Tertiary Qualified Physiotherapist with current AHPRA registration to support older adults in the City of Melbourne. The role focuses on assessing functional needs, setting care goals and delivering person-centred care in line with skilled standards.

You will work across home-based and community settings, conduct home visits, and collaborate with a multidisciplinary team to promote independence and active living for clients accessing Support at Home or CHSP services.
