Aussie Bridge Care is seeking a compassionate, reliable, and engaging Support Worker/Mentor to support a 17-year-old transgender female participant in the Boronia Heights area.

About the Role

Supports will be provided both in the participant's home and within the community, with a focus on:
Building confidence and self-esteem
Encouraging community participation and social inclusion
Supporting the development of daily living and independent living skills
Developing positive social connections
Providing mentoring and emotional support
Working collaboratively to achieve the participant's individual goals.
