Silverchain is seeking a Care Partner for our Aged Community Care Team in the South East Metro region. This full time, 12-month contract supports clients through the Support at Home program with a person-centred approach.

You will coordinate care, assess client needs, manage budgets and liaise with GPs and health specialists to deliver holistic, goal-focused care plans in a community setting.
