About the role The role is to manage a case load of clients under CHSP/HCP approved funding. Maintaining budgets, on boarding growth of recent clients and managing care requirements to maintain clients independency.

Liaising with stakeholders and internal networks

What you bring
Evidence of experience in a similar role
Relevant tertiary qualification in the community sector will be desirable
Demonstrated experience in the aged/community/disability sector particularly working with clients with complex needs
Full Drivers Licence and own reliable registered and insured Vehicle
Smart Phone
Knowledge of My Aged Care portal/assessment process
Willing to work in a team environment
A desire to join a team committed to the care of others in a mission based organisation
NDIS clearance (or willingness to obtain one)
Police clearance (within the last 6 months Or willingness to obtain one)
