This role focuses on empowering patients to manage chronic conditions and improve their overall health outcomes. We can entertain full time or part-time opportunities.

Key Responsibilities
Prepare Care Plans and Team Care Arrangements
Conduct health assessments and assist in early identification of chronic disease
Coach patients on health behaviour change principles
Collaborate with GPs and a multidisciplinary team to deliver preventative care
Manage administrative tasks related to Chronic Disease Management appointments

About You




AHPRA registered Enrolled or Registered Nurse
Experience in Chronic Disease Management and health assessments
Strong organisational and communication skills
A friendly, supportive approach and passion for patient care
