We are seeking a Case Manager to join our Complex Care Team, working within our Community Building Program .
About The Role
This is a site-based role delivering person-centred, trauma-informed case management services to residents at Park Towers, South Melbourne.
The role involves working with individuals experiencing complex and often chronic support needs, providing assertive engagement, advocacy, care coordination, and service navigation to enhance wellbeing, housing stability, social inclusion, and access to community supports.
You will support people to access both immediate supports and longer-term systems including NDIS and My Aged Care to promote sustained wellbeing, stability, and independence.
Park Towers: 9am – 5pm Weekdays only days as per your contract
This is hands-on, values-driven work that makes a substantial difference every day.
What you'll be doing
Deliver holistic, person-centred case management for clients with complex needs
Conduct comprehensive assessments and develop individualised case plans
Provide outreach and engagement in community-based settings
Advocate for clients across health, housing, legal and social support systems
Support clients to build capacity, independence and self-determination
Coordinate referrals and facilitate warm connections to long-term supports
Respond to and manage crisis situations with care and professionalism
Maintain high-quality documentation and contribute to program reporting
Work collaboratively with internal teams and external partners

What you'll bring
Tertiary qualifications in Social Work, Psychology, Community Services or a related field
Experience in case management with people experiencing complex and multiple disadvantages
Strong assessment, planning, advocacy and coordination skills
Excellent communication and relationship-building capability
Ability to think critically, manage competing priorities and respond in crisis
Understanding of the social determinants of health and service systems

About the Homelessness & Complex Care Team
The Complex Care Team is a highly specialised team of case managers who work with individuals from marginalised and vulnerable communities experiencing multiple and intersecting challenges.
Operating within a high-acuity environment, the team provides intensive, person-centred, and trauma-informed case management to clients with complex needs, including homelessness, mental health concerns, substance use issues, family violence, social isolation, and involvement with multiple service systems.
Through assertive outreach, engagement, advocacy, and coordinated care, the team supports clients to improve stability, wellbeing, safety, and access to appropriate services and opportunities.
