We're looking for an experienced and passionate Program Manager – Home Care to lead Home Care and Respite teams, ensuring exceptional, person-centred services that make a genuine difference in the lives of older people.
This is a key leadership role within the Home Care division, working closely with senior leadership to drive service excellence, support multidisciplinary teams, and deliver innovative, sustainable community care.
About the Role
As the Program Manager – Home Care, you will provide strategic and operational leadership to a small but dedicated team of aged care professionals.You will be responsible for ensuring high-quality, compliant and consumer-directed care while achieving operational and financial outcomes.
You'll also play a key role in workforce leadership, service development and continuous improvement.
Key ResponsibilitiesYou will:Lead and inspire multidisciplinary teams of professionals.Ensure high-quality, person-centred care that supports older people to remain independent at home.Oversee client assessment, care planning, service coordination and ongoing reviews.Drive customer satisfaction through responsive, respectful and goal-directed services.Manage budgets, resources and service performance to meet operational targets.Lead workforce planning, recruitment, coaching and performance development.Ensure compliance with the Aged Care Quality Standards, organisational policies and legislative requirements.Monitor quality, clinical governance,



risk management and continuous improvement initiatives.Build strong relationships with clients, families, community partners and referral agencies.Support the growth of fee-for-service opportunities across the region.
About YouYou are an experienced operational leader who is passionate about improving the lives of older people.
You will bring:Tertiary qualifications in Management, Social Work or Nursing or a related discipline, or equivalent leadership experience within health or aged care.Demonstrated experience managing community aged care or home care services.Strong knowledge of assessment, care planning and service delivery for older people living in the community.Proven financial and budget management experience.Experience leading, developing and mentoring multidisciplinary teams.Sound understanding of risk management, clinical governance and quality systems.Excellent communication, relationship-building and stakeholder engagement skills.The ability to lead change, improve services and foster a positive team culture.
