Role Description The Support Coordinator is a part-time role based in Prospect, SA, with a hybrid work arrangement that combines on-site client visits and office time with some work-from-home flexibility.
Day-to-day responsibilities include coordinating support plans, following up on client needs, and ensuring services are delivered in line with agreed goals and funding arrangements.
The role involves liaising with clients, families, and service providers, maintaining accurate records and case notes, and addressing service issues in a timely and professional manner.
The Support Coordinator will respond to inquiries, provide information about available supports, and assist clients in navigating systems, processes, and technology related to their care.
This role requires proactive communication, problem-solving, and regular reporting to ensure consistent, high-quality support outcomes.
Qualifications
Candidates should possess strong interpersonal and communication skills to build trust, explain information clearly, and work effectively with diverse clients and stakeholders.Candidates should possess case management skills to coordinate services, monitor progress, maintain documentation, and manage multiple client needs simultaneously.Candidates should possess customer service skills to respond professionally to inquiries, manage concerns, and provide a positive and respectful experience for clients and families.Candidates should possess technical support skills to assist clients with basic digital tools, portals, and systems used for scheduling, reporting, and accessing services.Relevant experience in community services, disability support, aged care, mental health, or a similar human services environment is beneficial.Certificate-level or tertiary qualifications in social work, community services, psychology, or a related field are desirable.Organizational and time management skills, with the ability to work independently, meet deadlines, and adapt to changing client needs, are key.Current driver's license, ability to travel within the local area, and any required background checks or clearances (such as working with vulnerable people) are advantageous.
