Are you an experienced Occupational Therapist who is passionate about helping older Australians maintain their independence, safety and quality of life at home?
We are seeking a dedicated Occupational Therapist to join our growing community aged care team, delivering high-quality, person-centred services across Melbourne's Northern and Western metropolitan regions.
This is an opportunity to make a meaningful difference through restorative care, reablement, assistive technology and home modification programs.
Based on a flexible hybrid work model, you'll split your time between our Brunswick office, working from home, and visiting clients in their homes.

$88,000 salary + 12% Super(pro-rata)
$8,000 Motor Vehicle Allowance
5 weeks annual leave

Weekly training and team meetings

Key Responsibilities
Conduct home-based occupational therapy assessments
Deliver reablement and restorative care programs
Recommend assistive technology and home modifications
Prepare clinical reports and funding documentation
Work closely with clients, families and the broader care team to achieve positive outcomes.
About You
Degree in Occupational Therapy
Current AHPRA registration
Minimum 3 years' community aged care experience
Knowledge of Home Care Packages and Support at Home programs
Current driver's licence and reliable vehicle
Excellent communication and report-writing skills.
