About the position
The Dedicated Intake Worker assists clients and potential clients when they contact Grampians Community Health with an enquiry, referral or support need.
The role provides a welcoming and person-centred first point of contact, ensuring individual needs are identified through Initial Needs Identification, appropriate information is provided, Initial Needs Identification is completed, and referrals or service connections are facilitated.
The position plays a critical role in ensuring timely access, understanding a person's immediate needs and priorities, identify their level of risk and priority of access to assessment, treatment, support and services, using tools such as the Initial Assessment and Referral Tool (IAR) and basic Alcohol and Other Drug screening.
The role is focused on access, coordination and navigation and does not replace program-specific assessment, clinical decision-making or ongoing case management.
The role operates in accordance with the Victorian Service Standards and the GCH Intake Practice Guide.
Skills and experience
Qualifications
• Certificate IV or Diploma in Community Services, Mental Health, AOD, Disability, Aged Care or related field
• Or two to three years' experience working in a relevant field and a willingness to attain further qualifications.
Mandatory
• Proven experience working in community, health or human services with people experiencing health, wellbeing or social support needs, and an ability to work within a strengths-based and person-centred framework.
Desirable
• Understanding and knowledge of services available within the region and beyond.
• Explicit understanding of GCH and its operations.
• Experience working within a community or human services setting.
