Highlights about the role at Australian Family Support (AFS)
As an energetic and experiencedSupport CoordinatorwithAustralian Family Support, you will play a vital role in assisting individuals with disabilities and older adults to navigate and access the supports and services they need.
This casual or part time role offers flexibility, allowing you to balance work with other priorities.
Your primary focus will be coordinating services, tracking their budgets to ensure continued services, providing regular updates on progress, empowering clients to achieve their goals, and ensuring they can lead fulfilling and independent lives.
What you'll be doing as core functions:
Developing, implementing, and reviewing individual support plans in collaboration with participants and their families/carers/advocates
Connecting clients with appropriate services, community resources, and funding options to meet their goals and needs
Advocating on behalf of participants to ensure their rights are upheld and their preferences are respected
Coordinating communication between participants, families/advocates, service providers, and other stakeholders to ensure seamless support and services
Monitoring progress and maintaining detailed records to ensure continuity and quality of care
Tracking their budgets and coordinating their expenses/invoices
Maintaining regular contact with support workers/carers/advocates and tracking participants' progress to understand any changes required and/or future needs
Staying up-to-date with relevant policies, procedures, and legislative requirements to ensure safe and compliant service delivery
What we're looking for in the ideal candidate:
A relevant qualification in disability, aged care, community services, social work, and relevant experience
A qualification in Social Work and current membership with the AASW are highly regarded
Experience in case management, support coordination,
and/or working with individuals with disabilities
Strong communication, interpersonal, and organizational skills, with the ability to build trust and rapport with participants
A proactive, problem-solving mindset, with a commitment to empowering participants and advocating for their needs
Flexibility and the ability to adapt support strategies to meet the unique needs of each participant
A current comprehensive car insurance and vehicle with NDIS compliant ANCAP rating (Desirable)
