We are currently seeking caring, reliable, and motivatedCommunity Support Workers / Home Care Workersto join our team.
This role involves supporting clients in their own homes and in the community to maintain their independence, dignity, safety, and quality of life.
The successful applicants will provide person-centred support to participants and clients with daily living tasks, personal care, domestic assistance, community access, and social support.
You will work closely with clients, families, and the care team to ensure support is delivered respectfully, safely, and according to each client's individual care plan.
Key Responsibilities
As a Community Support Worker / Home Care Worker, your duties may include:
Supporting clients with personal care, including showering, dressing, grooming, toileting, continence care, and mobility assistance.
Assisting with daily living tasks such as meal preparation, medication prompts, laundry, changing bed linen, and general household cleaning.
Providing domestic support to maintain a clean, safe, and comfortable home environment.
Supporting clients with community access, appointments, shopping, social outings, recreational activities, and participation in community programs.
Assisting clients to build independence, confidence, and daily living skills.
Following individual support plans,
behaviour support plans, manual handling plans, medication charts, mealtime management plans, and other care instructions.
Providing safe transfers and mobility support using appropriate equipment where required, such as hoists, slings, wheelchairs, walkers, or other mobility aids.
Monitoring client wellbeing and reporting any changes, concerns, incidents, hazards, or risks to the supervisor or care coordinator.
Maintaining accurate progress notes, shift reports, incident reports, and other documentation as required.
Promoting client choice, privacy, dignity, cultural needs, and independence at all times.
Working respectfully with clients, families, carers, allied health professionals, and other team members.
About You

You will have strong communication skills, a positive attitude, and the ability to work independently and as part of a team.
Essential Requirements
Certificate III or IV in Individual Support, Disability, Aged Care, Community Services, or equivalent is preferred.
Must Have Previous experience in disability support, aged care, home care,
or community support is highly regarded.
Current NDIS Worker Screening Check or willingness to obtain.
Current Working with Children Check, may required.
Current First Aid and CPR certificate or willingness to obtain.
Current driver's licence and reliable vehicle.
Ability to provide support in clients' homes and community settings.
Good communication, documentation, and time management skills.
Ability to follow care plans, workplace policies, and safety procedures.

Desirable Skills
Experience with manual handling and mobility equipment.
Experience supporting clients with disabilities, complex needs, dementia, mental health, or high personal care needs.
Medication assistance training.
Understanding of NDIS standards, person-centred care, and infection control practices.
