Registered Nurse – Community Nursing: Aged Care, NDIS and DVA

About the Role
As a Registered Nurse, you will provide clinical nursing care to clients in their own homes, supporting them to remain safe, healthy, and independent.
Responsibilities
Deliver meaningful, person-centred nursing care that makes a genuine difference in clients' lives
Use your clinical skills across assessments, care planning, medication support and wound care
Support clients with chronic conditions, diabetes management, continence care and post-hospital recovery
Provide compassionate palliative and end-of-life care when clients and families need it most
Work collaboratively with GPs, specialists and allied health professionals as part of a coordinated care team
Build trusted relationships with clients while supporting their independence, comfort and wellbeing
Maintain high-quality clinical documentation in a supportive and professional care setting
Apply infection prevention and clinical best-practice standards with confidence
Maintaining professional clinical records
Essential Requirements
Current AHPRA registration as a Registered Nurse
Current National Police Check
Current First Aid and CPR Certificate
Communication and documentation skills
Clinical assessment skills
Desirable
Community Nursing experience
DVA Community Nursing experience
Wound Care experience
Continence Management experience
NDIS Care experience
Palliative Care experience
Aged Care experience
